This notice explains how your personal information is handled when you contact Sophia Malik through this website. It's written in plain English and kept deliberately short.
Who is responsible for your information
Enquiries made through this website are received and handled by Sophia Malik, BDS (GDC No. 80003), who practises at Dental Wellbeing, Iver. You can reach her at sophiamalikdentistry@gmail.com.
What we collect
- Details you choose to send us, such as your name, email address, phone number, the treatment you're interested in, and your message.
- If you contact Sophia via WhatsApp, your message and phone number are processed by WhatsApp under its own terms.
Please don't include detailed medical or sensitive information in your first message, we can take that securely once you're in touch.
Why we use it, and our lawful basis
We use your details only to respond to your enquiry and arrange an appointment. Our lawful basis is your consent and our legitimate interest in responding to people who contact us. If you go on to become a patient, your clinical information is handled under the practice's separate patient privacy notice.
How the contact form works
The contact form is delivered using a third-party form service (Web3Forms) which forwards your message to Sophia by email. Your data passes through that service only to deliver your message. Please see the form provider's own privacy terms for details.
How long we keep it
Enquiry messages are kept only as long as needed to deal with your request and for a reasonable period afterwards, then deleted. If you become a patient, clinical records are retained in line with the practice's policy and professional requirements.
Who we share it with
We don't sell your information or use it for marketing. It may be processed by the email and form providers used to operate this site. Where you've consented, your appointment may be arranged with the practice so it can be booked into the diary.
Your rights
You have the right to ask for a copy of your information, to have it corrected or deleted, and to object to or restrict its use. To exercise any of these, email sophiamalikdentistry@gmail.com. You can also complain to the Information Commissioner's Office (ICO) at ico.org.uk.
Cookies
This website uses minimal essential resources and does not set marketing cookies. Embedded content (such as Instagram videos) may set its own cookies when it loads; these are controlled by those third parties.
